Factual Errors Report

The Assessment Guide has multiple recurring errors that include more than 1 correct answer choice.

Teachers are directed to say, "Listen to the sounds in the word 'these'." Then the phonetic pronunciation is indicated as /th- e-s/, which is incorrect. On a previous page in the Teachers' Guide the sounds were represented correctly as /th-e-z/. The /s/ needs to be revised.

Question 5 in 'Spiral Review Assessment 1: Phonological Awareness' asks students how many syllables are in the word 'elephant' with the correct answer being four. That is incorrect. There are three syllables in the word 'elephant.'

The question states, "Which word has 'p' as the last sound?" Two answer choices with the last sound of 'p' are provided (map, tap). However, the answer key incorrecly only identifies the word 'map' as the correct answer.

Question #2 asks, "What word do you get when you blend the sounds /b/ /e/ /ch/?" The /n/ is omitted. However, the answer key identifies 'bench' with the letter 'n' as the correct answer. The /n/ needs to be added to the question's stem.

Question 5 states, "What word do you get when you add /ch/ to the end of the word 'hunch'? The answer choices are a) hunchback b)hutch c)hitch d)crunch. The correct answer would be nonsense word "hunchch", which is not listed on the answer key.

The questions states, "Which of the following words has the same ending sound as 'splash'? " These answer choices are given: a) brush b)torch c)fresh d)wish. The answer key provides the word 'fresh' as the correct answer. However, the words (brush and wish) also have the ending sound /sh/. This question needs to be revised.

The question asks, "Which of the following words has the same ending sound as "earth"? The words 'path' 'mouth' and 'width' are three correct answer choices, however the answer key only lists one word as the correct answer ('width').

The question asks, "Which of the following words has the same ending sound as "fourth"? The words 'north' 'path' and 'mouth' are all correct answer choices, however, the answer key only lists one word ('north') as correct.

The question asks, "Which of the following words has the same ending sound as "block"? The words 'rock' 'wreck' and 'flick' each share the same ending sound as the word 'block'. The answer key lists only one correct word ('rock').

The question asks, "What word do you get when you remove the /n/ from "knight"? The answer key reveals the correct answer to be the word "right" but that is not one of the available answer choices. The correct response would be 'ight' when removing the sound /n/ from the word 'knight'.

Question #4 says, "Which of the following words has the same ending sound as "bright?" These answer choices provided are swift, grip, light, and give. The answer key only list (c) light as correct, but there are two words with the ending sound /t/. I suggest revising either re the question or answer choices.

The question asks, "Which word has the same vowel sound as "rope"? Of the answer choices provided, the words 'row' and 'road' both have the same vowel sound as the word 'rope'. According to the answer key, the correct choice is the word 'road' and that is not true.

The usage of that punctuation in this question (specifically as answer choices) is misleading and incorrect. The answer choices should be written without the forward slashes to indicate letters.

The question asks, "Which word has the same vowel pattern as "soap"? There are two correct choices ('goat' and 'goal'), however, the answer key lists the word 'goat' as the only correct response.
